2009 REGISTRATION DATES

In-person on-site Registration at our field will take place on the following dates:

• Wednesday, April 1: 6 – 8 PM
• Saturday, April 18: 10 AM – 12 Noon
• Wednesday, April 29: 6 – 8 PM
• Saturday, May 9: 10 AM – 12 Noon
• Wednesday, May 20: 6 – 8 PM
• THIRD ANNUAL FUN DAY – Saturday, June 13: 1 - 4 PM (including free games, food and more)

WHAT YOU NEED TO BRING

• Registration & Code of Conduct Form (completed)
• Form is available under the "Registration Information" tab
• Physical Form completed by the child's Physician
• Form is available under the "Registration Information" tab
• Copy of the child's Birth Certificate
• Copy of the child's LAST report card
